To address a growing interest in assessing patient experience with the patient-centered medical home model, the CAHPS Consortium has developed an expanded version of the Clinician & Group 12-Month Survey that incorporates the CAHPS Patient-Centered Medical Home Item Set. Although this survey could be used by any physician practice, it is expected to be especially useful for physician practices that have adopted features of a patient-centered medical home (PCMH). The Agency for Healthcare Research and Quality released this expanded survey in October 2011.

The expanded survey includes standardized questionnaires for adults and children in primary care settings.

Languages. The questionnaires and supplemental items are available in English and Spanish. For help in producing questionnaires in other languages, please go to Translating Surveys and Other Materials.

Use by NCQA. The National Committee for Quality Assurance’s Patient-Centered Medical Home 2011 program is using this expanded 12-Month Survey with PCMH items for its optional Distinction in Patient Experience Reporting. Information on this program is available at

Quality Measures from the 12-Month Survey

The expanded survey includes all of the standard patient experience measures for the Clinician & Group Survey:

  • Getting Timely Appointments, Care, and Information
  • How Well Providers (or Doctors) Communicate With Patients
  • Helpful, Courteous, and Respectful Office Staff
  • Patients' Rating of the Provider (or Doctor)
  • Provider's (or Doctor's) Attention to Your Child's Growth and Development (child only)
  • Provider's (or Doctor's) Advice on Keeping Your Child Safe and Healthy (child only)

It also offers three new composite measures. Please note that these provisional labels for the measures are being tested with consumers and may be revised:

  • Providers Pay Attention to Your Mental or Emotional Health (adult only)
  • Providers Support You in Taking Care of Your Own Health
  • Providers Discuss Medication Decisions (adult only)

In addition, the survey includes items that address coordination of care and information about care and appointments, but those items should not be reported as composite measures.

Brief History

As with all other CAHPS surveys, the expanded 12-Month Survey with PCMH Items has undergone a rigorous development process that includes multiple steps:

  • Literature review.
  • Input from a Technical Expert Panel and other stakeholders.
  • Focus groups.
  • Cognitive testing in English and Spanish.
  • Development of a survey for field testing.
  • Field testing of the draft version of the PCMH Survey.
  • Analysis of the field test results and refinement of the survey.
