To assess patient experience with the patient-centered medical home model, the CAHPS Consortium developed an expanded version of the Clinician & Group 12-Month Survey that incorporated the CAHPS Patient-Centered Medical Home (PCMH) Item Set 2.0. The Agency for Healthcare Research and Quality released this expanded survey, known as the CAHPS PCMH Survey, in October 2011 as part of the 2.0 version of the Clinician & Group Survey.

The PCMH Survey includes standardized questionnaires for adults and children in primary care settings. Although this survey could be used by any physician practice, it is intended to be especially useful for physician practices that have adopted features of a patient-centered medical home.

Quality Measures from the PCMH Survey

The PCMH Survey 2.0 produces the same set of patient experience measures as the 12-Month Survey 2.0:

  • Getting Timely Appointments, Care, and Information
  • How Well Providers Communicate With Patients
  • Helpful, Courteous, and Respectful Office Staff
  • Patients' Rating of the Provider
  • Provider's Attention to Your Child's Growth and Development (Child only)
  • Provider's Advice on Keeping Your Child Safe and Healthy (Child only)

It also produces three additional composite measures:

  • Talking With You About Taking Care of Your Own Health
  • Attention to Your Mental and Emotional Health (Adult only)
  • Talking About Medication Decisions (Adult only)

Use of PCMH Survey by NCQA

The National Committee for Quality Assurance’s Patient-Centered Medical Home program uses the CAHPS PCMH Survey for its optional Distinction in Patient Experience Reporting.
